What To Expect from An Eyelid Surgery with Dr. Paydar

Dr. Paydar conducts all of his surgeries at his AAAASF accredited state of the art facility, Newport Aesthetic Surgery Center, under general anesthesia with a board-certified anesthesiologist present for the duration of the surgery. Blepharoplasty is generally broken down into two sub-procedures: the upper eyelids and the lower eyelids. 

When addressing the upper eyelids, Dr. Paydar will first make an incision above the eyelid. He will then visually locate the excess skin and confirm that by measuring the skin with a medical instrument. When he decides how much skin he needs to remove, he’ll carefully excise the excess skin and fat from the same incision. When Dr. Paydar is satisfied with the appearance of the eyelids, the incisions will be sutured and dressed for optimal results.

For the lower eyelids, Dr. Paydar will make an incision below the eyelashes that can be camouflaged almost immediately after surgery to remove any excess skin. The incision will be sutured and dressed for recovery. The fat that produces the under-eye bags can be addressed by performing an incision in the conjunctiva, or the inside lining of the lower eyelid, followed by the release and repositioning of the fat on the cheek to soften the border between the lower lid and the cheek fat. There is a good chance that the laxity in the muscle of the lower lid may require tightening with a stitch which will be done prior to the final simple stitch in the lining of the lower lid. The lower lid skin will then be supported with tape.

What To Expect from Eyelid Surgery/ Brow Lift Surgery Recovery

The recovery from eyelid surgery or brow lift is one of the easiest. You’ll feel back to normal around the second day of your recovery. The healing process generally takes about a week for the upper eyelids and about 10-14 days for the lower eyelids.

Your brow lift recovery process is approximately 10 to 14 days, similar to lower eyelid recovery.

It’s important to limit excess physical activity until you are fully healed and cleared by your doctor. You want to protect your incisions from sweat or accidental trauma by attending the gym too early. You must also refrain from putting any makeup or cosmetics on over your incisions until they are fully healed to stave off any infection.

Excess skin, fat, or droopiness, asymmetry, tear trough deformity, laxity of the lids and loss of tilt of the eyes can all be addressed.

Depending on if it’s an upper eyelid surgery only which can be done under local anesthetic or if it’s a lower lid operation which will be done under general anesthesia, you will still be able to go home within hours after the start of the operation.

This location on the body does not cause thick hypertrophic or keloid scars.

That is the goal each time this operation is performed is to improve the aesthetics and symmetry.

Yes, the change in the eyes will be a refreshing and more youthful one.

It’s a possibility only if a major asymmetry exists, but it’s not common.

An incision is made around the excess skin and it is removed along with a strip of muscle and possibly some fat in the upper eyelids.  For the lower lid, an incision is made in the conjunctiva and the excess fat is released and placed under the trough to soften up the depression there.  Excess skin is removed conservatively below the lower lash line and if laxity exists to the lower lids, adjustments are made to the lateral canthus which also helps against swelling.

We don’t offer any laser technique as the conventional technique gives the most improvement.

For minimal wrinkles or excess skin, it helps achieve small improvement without creating the incisions or causing deep scarring.

Lasers can cause pigmentation changes in those with darker complexion.  Topical pain medication is usually applied prior to laser.

There is a high rate of success with a natural rejuvenation of the eyelids.

The wrinkles in the Crow’s feet will be improved, but not eliminated as the muscle controlling the creation of the wrinkles is not removed. The droopy eyebrows can be treated at the same time with brow lift. The dark circles under the lower lids improves with fat grafting that can be done at the same time.

The bags in the lower lids created by the excess fat in the lower lids that are separated by the sagging cheeks can be addressed by repositioning them onto the tear troughs. This can improve the discoloration in the area. Fat grafting can also help with the dark circles.

The position of the brow will be evaluated during your consultation and can be addressed during the eyelid operation. If a brow lift only improves the upper lid aesthetics, then an upper blepharoplasty is avoided.

Everyone has a unique circumstance when it comes to aging of the face. Some require or desire an eyelid lift before a facelift.  Some have both areas treated at the same time.

As long as you are healthy, patients between the age of 40 to late 80’s undergo this operation.

Yes, some patients lose their upper field vision with excess upper eyelids skin hooding over the lashes.

Scars are forever, but the blepharoplasty scars are not noticeable due to the placement of the incisions.

Scarring, asymmetry, hematoma, and possible lower lids malposition are some of the possible side effects.

Incisions are placed where they will be concealed easily.  For the upper lids, an incision in the prominent crease extending out towards the crow’s feet and for the lower lids it can be on the inside in the conjunctiva or the lining or outside just under the lash line.

By two weeks, most of the swelling and bruising will have resolved and by three weeks everything will be near normal and makeup is possible.

The incisions are a standard part of the operation and the decision is made based on areas that need improvement.

There is no stopping the aging process. So improvements can last a long time as long as one takes good care of the skin against elements like sun or smoking.

Depending on the type of anesthesia (local versus general), you can walk out without any issue or may need someone to take you home and watch over you the first night.

Upper eyelids can be done just under one hour and the lower lids are usually more complicated and can take twice as long.

The operation can be one to three hours, depending on if there is need for fat grafting as well, but the healing takes about two weeks.

If having upper blepharoplasty under local anesthesia, it is possible.  However, it is always prudent to have someone be with you in case any complications occur the first night.
