What Is a Facelift Surgery?

Facelift surgery is a cosmetic procedure that helps to recreate more youthful facial contours. It reduces sagging skin, deep-set lines and wrinkles around the jaw, cheeks, and eyes, lifts the jawline to help eliminate jowls, lifts the cheeks to recreast a more youthful “ogee curve,” and restores a smooth complexion. Medically known as a rhytidectomy, a facelift procedure repositions muscle fascia tissue and removes loose skin to address common signs of aging in the face, including:

  • Prominent hollows beneath the eyes (when cheek fat is lifted)
  • Jowls
  • Loose facial skin
  • Sagging cheeks
  • Skin folds

Dr. Paydar may recommend a fat transfer to recreate a fuller, more youthful appearance. Starting at the age of 30 people start to losing about 1% of their facial fat per year, which is why we start looking hollower with age. Additionally, what facial fat we have left starts to fall with gravity. The facelift will help to lift what fat has fallen on the lower part of the face and the cheeks.

What Are the Benefits of a Facelift?

Wrinkles and sagging can make us look older than we feel. Now, you can refresh your appearance by repositioning facial skin, muscle fascia, and outer tissues. No one can say how many years it will take away from your appearance, but it will restore a natural, beautiful, and youthful look.

As we age, we lose essential proteins responsible for keeping our skin firm and supple, like collagen and elastin. As the production of these compounds begins to decrease, our skin starts to sag. During a facelift, Dr. Paydar will lift the cheeks to improve the overall contour of your face and restore definition to your cheekbones (restore the “ogee” S-shaped curve) and jawline. These simple enhancements can make you look younger and livelier.

Dr. Paydar’s goal is not to change the structure of your face but rather to restore or enhance it. He is not aiming to make you look “windswept” or artificial—by using his skill and advanced techniques during facelifts, he can improve your natural beauty rather than dramatically alter your appearance.

How Does Dr. Paydar Perform Facelift Surgery?

Dr. Paydar conducts all of his surgeries at his AAAASF accredited state of the art facility, Newport Aesthetic Surgery Center, under general anesthesia with a board certified anesthesiologist present for the duration of the surgery. The techniques Dr. Paydar chooses to use will depend entirely on the individual facial structure of the patient, the extent of the patient’s desired corrections, and how safely and efficiently Dr. Paydar plans to meet your aesthetic goals and expectations.

Typically, facelift procedures involve incisions placed in front and sometimes extended to behind the ear where Dr. Paydar is able to access the facial muscle fascia and tissue that need to be carefully adjusted. Any excess skin will be removed and stretched lightly to provide a naturally lifted appearance and assist in smoothing lines and wrinkles. Once the facelift surgery is complete, he closes incisions with stitches, hiding them where they’ll heal discreetly in the hairline and natural facial contours around the ear.

Can A Facelift Be Combined with Other Procedures?

Age can significantly affect our facial features—our eyes, cheeks, chin, neck, etc., will age, and we will stop looking like ourselves. While a facelift offers significant rejuvenation to the facial features, it cannot address everything. Many facelift patients seek additional procedures like eyelid surgery, brow lift, or a neck lift for total facial rejuvenation.

Drooping eyelids, though common signs of time and age, can cause wrinkles, vision impairments, bags, and the appearance of being tired. Visible signs like these can be reversed with an eyelid surgery that removes excess skin and fat from above and around the eyes.

Brow lift surgery addresses the upper portion of the face, from the brow line to the forehead. If you have a heavy brow and have begun to appear angry, tired, sad, or some other emotion you are not, brow lift surgery can lift the tissue and skin in this region to make you appear younger and restore your expressions.

A neck lift can significantly diminish signs of aging around the neck by eliminating excess fat, tightening the underlying muscles, and lifting the skin and tissue to a more naturally youthful position. Facelifts are often done in conjunction with a neck lift for a more harmonious restoration and balance of facial features.

Learning about facelift by resarching is always encouraged. It is important to let your facelift surgeon know what aspects of your face bother you the most and how far you would be willing to go to get all around harmonious results as other areas could need treatment as well, with the neck being the most common other site that may need a lift and tightening.

Most patients fall in the range of 5th to the 7th decades of their lives, but we do get some in their thirties or in their eighties that may require minor procedures or touch ups as well.

You will be given prescribed medication prior to your operation to keep you calm and have your blood pressure under control before, during, and after surgery.  You should have purchased household items ahead of time, so you don’t need to leave the house.  You should have selected someone you trust to be with you the day of surgery for the first 24 to 48 hours preferably.  You will not be able to drive after surgery so make sure someone will be able to drive you home or to a comfortable retreat.

Depending on patient factors and other areas that are being lifting or treated at the same time (such as a neck lift), it can take anywhere from two to six hours under the supervision of a board certified anesthesiologist.

The decision ultimately belongs to the patient.  Some go out within days for routine grocery shopping.  The swelling is worse two to three days after the operation and we place drains to minimize the bruising and swelling the first few days.  After that, going into a more manageable face and neck compression allows quick transition to take off the wrap during short periods when in public.  Most swelling and bruising is gone by 10-14 days and most patients feel more comfortable to go out in public at that time.  Makeup over the incisions are usually allowed at three weeks after surgery.

Depending on patient factors such as age, sun damage, smoking history, and genetics, it can last about 10-15 years.

Sun damage, age, genetics, and tobacco/nicotine are the usual factors that affect the lasting of results.  Those with little sun damage, who have protected their skin for years, and have abstained from smoking and get their face and necklift at an earlier age will have longer lasting results.

A facelift that treats the underlying planes below the skin, known as the SMAS, is a deep plane facelift.  Manipulation of the SMAS allows us to create  fuller cheeks and eliminate sagging jowls and improve the nasolabial and marionette folds.  The skin can then be lightly pulled to improve the wrinkles.

SMAS is the deep layer beneath skin that is connected to the neck and forehead and the scalp that when detached and lifted or plicated, it can recreate the more natural lifted look beneath the skin.  It is often pulled in a different vector than the face to allow a natural beautiful finish.

Traditional facelift implies a more significant surgery where many planes in the face are addressed with a longer recovery, but with a longer lasting results usually.  Mini-facelift has been associated with those who are not willing or able to provide a full operation for reasons such as lack of skill or patient desires for a faster recovery not to include possible monetary reasons.

When a patient is a candidate for a facelift procedure, often other non-surgical options are not the best choice or could prolong the inevitable with less than acceptable results.

Liquid facelift is a marketing campaign for minimally invasive procedures such as fillers or fat grafting for example.  They address the deflated areas of the face that occur with aging, but do not treat the sagging tissue very well and looking unnatural from such procedures is common.

Usually it takes two to three weeks to get back to normal where public appearance is comfortable.  Swelling can last for months and scars take three to six months to heal and fade away nicely.

You can expect a refreshed and a natural look with the method of facelift we offer.  Too tight of a skin pull to address the aging face will not deliver a natural beautiful result.  By addressing the deep facial planes and gently pulling on the skin can give you the best natural results in the hands of a properly trained and experienced board certified plastic and reconstructive surgeon.

Most swelling and bruising resolve at 10 to 14 days after surgery.  Most patients feel comfortable going back to work at two to three weeks after a facelift.  It is important to wear the face and neck compression for a minimum of three weeks as much as possible.  Thus working part time to allow maximum compression is helpful early on.

Walking is encouraged early on as in days after surgery. Cardio exercises can start around three to four weeks after and heavy duty lifting at about six to eight weeks post procedure.

You can predict a complication rate of 100% with facelift if you smoke.  Healing is affected in those who smoke tobacco or vape nicotine as not enough oxygen or healing tissue are delivered to thinned out tissue with minimal blood supply that needs to heal after a facelift.

Decreased sensation to the face and facial nerve injury that may result in a transient asymmetrical smile or facial expression are possible but not very common.  The worst thing that can happen which is very uncommon is bleeding or hematoma that can put pressure on the trachea, but we are diligent and methodical at the time of the operation to stop any bleeding.

Swelling and bruising are common after a facelift procedure.  Inability to feel the sensation on the skin occurs early on and gets better over time.  Scarring is necessary for a facelift but they will be hardly noticeable as there are areas chosen appropriately to hide scars along the front of the ear using the contours available and extending to hairlines or behind the ears.
